Forms and Agreements
Do not make substantive changes to these documents without OTC approval. Agreements are binding only if approved by OTC staff. Please contact the person listed for copies of these forms. You may also contact your case manager at (765) 494-2610 for any help you may need concerning these documents.
Permission to Engage in an Outside Activity, Form 32A – This form must be obtained from the business office at your school or department.
Pre-Disclosure Form – This form is the first step in the technology transfer process and must be completed when you feel you have an idea that is patentable and has market potential. Sending in this form will prompt a search of the field and a pre-disclosure meeting OTC licensing associate to discuss the invention and collectively define the next steps in the process.
Invention Record and Disclosure Form (IRD) – This form is used for disclosing intellectual property to the Office of Technology Commercialization. The disclosure document is to be prepared by the inventor(s) and submitted to OTC. It provides the basis for a determination of patentability and for drafting a patent application. Confidential Disclosure Agreement (CDA) – This form is used for disclosing unpublished, unpatented information to a third party. A CDA is designed to maintain the confidentiality of ideas, inventions, etc. It must be signed by all parties involved. Material Use Agreement (MUA) – This form is used for transferring Purdue University materials to a third party. Contact: Agreement Coordinator, (765) 494-5820
