Permission to Engage in an Outside Activity, Form 32A – This form must be obtained from the business office at your school or department.

Pre-Disclosure Form – This form is the first step in the technology transfer process and must be completed when you feel you have an idea that is patentable and has market potential. Sending in this form will prompt a search of the field and a pre-disclosure meeting OTC licensing associate to discuss the invention and collectively define the next steps in the process.

Invention Record and Disclosure Form (IRD) – This form is used for disclosing intellectual property to the Office of Technology Commercialization. The disclosure document is to be prepared by the inventor(s) and submitted to OTC. It provides the basis for a determination of patentability and for drafting a patent application. Confidential Disclosure Agreement (CDA) – This form is used for disclosing unpublished, unpatented information to a third party.  A CDA is designed to maintain the confidentiality of ideas, inventions, etc. It must be signed by all parties involved. Contact the Office of Technology Commercialization before you discuss your ideas or invention with anyone outside the university. Contact: Agreement Coordinator, (765) 494-5820

Material Use Agreement (MUA) – This form is used for transferring Purdue University materials to a third party. Contact: Agreement Coordinator, (765) 494-5820

In order to ensure the security of your personal information, the following two forms must be printed out and then hand-delivered to Accounting Division, Purdue Research Foundation:

Certificate of Foreign Status of Beneficial Owner for US Tax Withholding (W-8BEN)
